There are three bolts that hold the manifold to the cat and the bottom bolt sheered off and I can't get the stud out. Any ideas? I'm wondering if that small exhaust leak is what's triggering my famous SVT Focus left bank running lean check engine light.

You can either drill out the hole, starting small and gradually getting bigger. Or you can pull the header and tap it out with a centerpunch and hammer. I recommend replacing all three bolts, as well as the gasket.

How hard are they to punch out? Once I get the header off, I'm assuming I can punch it out while the cat is on the car?

Once you have the cat out, you should be able to tap them out with a punch and hammer.
