It could be a T-Connector Engine Coolant Hose, picture below. However it looks like yours is metal. I doubt they make the metal ones anymore, probably upgraded to plastic. I would remove it first before you get another one to verify the fit of the new one. I would check junk yards if you want a metal one.

Is the side of the tee that you can't see threaded into something or is it a hose and clamp like the other two sides? If it's a hose and clamp take the tee out go to someplace like Grainger or even Home Depot and see if they have a hose barb tee or a brass tee that you can thread 3 hose barbs into.

 

That's what we wound up doing. Got a 3/4" heater hose tee. So far it's working.
