i had a somwhat similar issue, my hfc rear O2 bung just broke right off at the pipe. it was wierd... got it welded back on but now it sounds different...

looks terrible... thankfully i kept my stock cat and with an hour and air tools had it replaced.

Please keep in mind that anything is prone to failure... We have over 200 high flow cats out there and only have had to warranty a few.
