Automatic Gearbox not changing from 2nd to 3rd till 4000rev

Hello forum. I have developed a problem with my gearbox a cpl of days ago. I have a 2004 Focus Automatic. When I put it into drive there is a clunk and and the car jerks. and from 2nd to 3rd gear it dosent change gear till around 4000revs. I have topped up the trany fluid but to no effect. Has anyone else had the same problem or dose any one know what's causing it?
