Well got some interest in this and actually found this a few weeks ago in my basement. Figured someone might want it. I believe this is the last one I had made up from years ago. Too long ago haha.

Thanks for opening it back up.
Well lucky for one special person, I found one last one. I'm 100% this is the last one. Little dirty but should clean up for paint just fine.  Is also trade for car stuff it doesn't bother me. I may also just keep it for myself if I ever decide to put my switches in there.
