I have the same exact issue. When I took mine in the first time, it looked like they had simply crimped the fabric liner back onto the channel, and it held for about two months. I had to fold my rear seats down the other day and noticed that it has come off again. I need to take my car back in for some other warranty work and will see how they fix it this time.

I've had my Focus Titanium for five months and yesterday was the first time I folded down the rear seats and turns out I have the same problem! I'll be going in for my first oil change so I'll be pointing this out to the dealer as it has to be fixed. Glad to see I'm not the only one with this problem but at the same time not happy that it's like this. I did not cause this and it's a factory issue so when I read that the dealer said it was the owner's fault for pushing the seat down too far that truly is a load of crock. Hopefully I'm not told the same.

I was at the dealership today to have my tires switched over. They had two guys trying to slip it back in and when they folded the seat down it popped back out. They ended up saying that the only way it could be fixed would be to order a whole new piece and I'm good with that. At least I'll know it'll be fixed for good!

Yeah Crystal I gotta say the dealer looks after me. Right from the sales lady who sold me the car to the service manager. I'd recommend these guys in a heartbeat.
