www.FocusSport.com sells them as well as www.massivespeedsystem.com and www.c-f-m.com. Replacing all three motor mounts with poly ones will cause some crazy vibration on ur car. Go to a dealer and find some OEM replacements. All three poly are quite hefty in price, thats if u can find them for an SPI(nobody that i know of sells all 3 for the SPI). If anything just buy a rear poly motor mount and the other ones replace them with new stock ones.

How about some help from someone who actually has experience in this subject.
I currently have the ONLY spi with all 3 motor mounts.
The lower motor mount is universal between all focus engines,
and the zetec drivers side motor mount fits the spi,
Here's a pic of both of mine (lower mount is the one on bottom of pic):

and the passenger side zetec mount does not work on the spi, as seen in this pic:

so I had HTPerformance design and fabricate me one.
Here's pics of it:

They will make any spi owner one if you'd like one.
The motor mounts make a significant difference, and they are well worth it.
The vibration even with all 3 mounts isn't bad at all. If you'd like to see installed pics, let me know.

As far as the axles go, maybe someone else can help you out there, since ive never had to change mine.
