I had this problem. Dealer replaced the fuel filler neck. Everything works now. I think someone was trying to siphon my gas (I had left the car unlocked that night by accident). Whoever it was actually siphoned about 10 gallons of gas out of my wife's Xterra (picked up a locking gas cap the next day) ...

Is it a freak thing or has other people had this problem? Ill see if it does it on next fill up.
I had this problem, only worse, with my old Elantra - I could only trickle in gas. I asked the dealer if they had an idea and they said they might, bring it in and we'll look it over - uh, huh, I don't think so. A little research and I discovered that the tank vent filter - conveniently located in the left rear wheel well, was plugged with dust and mud. I bought a new one for $6, pulled the wheel, popped it in, re-shoed the beast and never had a problem again. Unfortunately, it appears that the tank on the Focus vents in a different manner as I don't see a filter on the fuel system schematic - but it could easily be a vent issue.

It doesn't matter if the car was unlocked or not, the fuel door is never locked. Although if someone did try to siphon the fuel without a fuel nozzle sized tube (like the one stored with the spare wheel in the epp foam spacer underneath the floor in the trunk/ hatch area) all they could likely do is damage things.

I filled up last night and all was good. But, This time the gas station was very busy and the pumps seem to be going slow.

That sucks. I know when gas was over $6 gallon here a few years ago hardware stores were running out of cordless drills as thieves were just drilling the gas tanks. I hope manufactures concentrate on fixing this in future models.

A long awaited update: I took the car in for an oil change and mentioned the issue. They checked evap, charcol canister etc. Found everything to be in working order. They blew the vent line out. Happy to report that there must have been a clog in the vent line. When I fill up after that it was a big relief!!

Sometimes the vent gets frozen over with ice. That can cause what you are describing.
